I have used this questionnaire for the past 14 years, and I absolutely love it! There are 15 "get to know you" questions for the students to complete. There are some fun questions where you can get to know their personalities, and there are also some serious questions so you can be in the know about their home lives, too. I have discovered some great info about kids and how to teach and approach different problems with them from this survey!
